DOMICILIARY CARE
We are a care provider that aims to deliver high-quality, safe, and effective service. Our care service supports you with washing and dressing tasks, getting in and out of your bed, administering medication, meal preparation, sit-in services, companionship, wake-night services, shopping, errands, light housework, toileting, and continence care. We promote choice, and independence and maintain an individual in an acceptable level of health, hygiene, dignity, safety, and ease in their home. We promote personalised care and one-on-one attention, allowing carers to establish stronger relationships with you and gain a deeper understanding of your needs. Our discreet care at-home service is designed to support you with your day-to-day living, as well as providing you with the encouragement and emotional support that you may need to remain living independently. |
SUPPORTED LIVING
We provide supporting care so you can live in your own home. We work in partnership with you and agree together on how we will support you to be as independent as possible, both in your own home and in your local community. We also involve your family and any practitioners who might be assisting you. You will receive emotional and physical support from a specialist staff team who are trained to meet your specific needs. We carefully match our team members to each person so that we can nurture trust and build positive relationships. With individually tailored support from people, they feel comfortable and secure with, tenants quickly grow in confidence. We provide help across a range of areas to enable individuals to lead more enriching and fulfilling lives. This includes: Social inclusion - accessing and attending community-based activities. Domestic tasks such as cooking, cleaning, and shopping Personal care Taking medication Managing finances and budgeting 24-hour on-call support service for the support team Supporting with finding daytime activities and employment/voluntary work Helping with claiming and managing benefits Accessing leisure and social activities Maintaining links with family and friends, and establishing new relationships Supporting healthy lifestyles |
LIVE-IN CARE
Live-in care enables elderly or younger adults with care or extra support needs to continue to live in their own homes with 24 hour one-to-one care and support from a carefully matched care worker. We understand your loved one’s safety is paramount. That’s why we go to great lengths to ensure their carer is someone the whole family can trust. All the professionals we work with have background checks, referencing and certificates to prove they reach the standards you expect. All our care workers have nationally recognised qualifications and are enhanced DBS checked |
MENTAL HEALTH
Greenmore Healthcare services provide support and care for individuals living with mental health conditions such as Depression, schizophrenia, anxiety, phobias, and Personality disorders to enable them to live a fulfilling life with greater independence and in the least restrictive environments in the community. We offer skilled, personalized care and support individuals in their journey of recovery using the recovery model – recognizing that everyone’s journey to recovery is different. We support individuals with different mental health needs to engage in meaningful activities and work towards achieving their outcomes and goals. We support individuals to maintain compliance with medication, community treatment orders (CTO), and Ministry of Justice (MOJ) conditions and restrictions. We support individuals with services that include activities of daily living, personal care, accessing and attending community-based activities, support to attend community-based education/training settings, budgeting skills, shopping, going on holiday trips, sitting in companionship, feeding+ dog walking, maintaining healthy lifestyles, and visiting friends and family. All our support is carefully planned collaboratively with each person, their family, and care professionals, ensuring that everyone's voice is heard and that the plan is tailored to reflect all needs and wishes and reduce the risk of unnecessary hospital admissions. |
Learning Disability and Autism
Our care and support is all about you. We pride ourselves on creating tailored plans that evolve with your needs and enhance your quality of life. We use a person-centered approach- it ensures that the people we support are able to develop the knowledge, skills, and confidence they require to manage and make informed decisions about their own lives, and wellbeing - treating them as equal partners at all times. We use Positive Behaviour Support (PBS) to support individuals who may be described as having challenging behaviours. PBS enables individuals to communicate and get the life they want by creating a system of support that values dignity, choice, participation, equal opportunity, and taking positive risks in a well-planned and safe manner. We are happy to work with individuals under the Transforming Care pathway and on the Dynamic Support Pathway (DSP) Register. |
